ARSD § 64:13:01:05 — Air pollution sources -- Annual report required

64:13:01:05. Air
pollution source -- Annual report required. An air pollution source
required to pay the fee provided for under SDCL 34A-1-58 shall file a report on forms which include the following:
(1) The name and address of the air pollution
source;
(2) The name of the person to be contacted in
regard to the report;
(3) The type of source operated by the filer;
(4) The signature of the principal of the filer;
(5) The computation of the fee due under SDCL 34A-1-58 ; and
(6) Any other information that permits the
department to determine whether the fee is properly computed and reported.
