64:13:01:01. Wastewater fee forms -- Publicly and privately owned treatment works -- Annual report required. A facility required to pay fees pursuant to SDCL 34A-2-117 shall file an annual report on the forms which include the following:

# (1)

The name and address of the treatment works;

# (2)

The name of the person to be contacted in regard to the report;

# (3)

The type of system operated by the filer;

# (4)

The computation of the fee due under SDCL 34A-1-117; and

# (5)

Any other information which allows the department to determine whether the fee is properly computed and reported.
Additionally, publicly owned treatment works, including municipalities, housing subdivisions, trailer parks, and sanitary districts owned by the state or a political subdivision of the state, or Ellsworth Air Force Base shall report their population served. Privately owned treatment works, including housing subdivisions and trailer parks which treat primarily domestic household wastewater shall report their population served.
