K.A.R. 92-13-1
Application: contents.
All applications for an interstate motor fuel user license shall be on forms furnished by the director and shall contain information as follows:
# (a)
The name and address of the applicant.
# (b)
The principal office or place of business of the applicant.
# (c)
If a corporation, the names and addresses of the principal officers.
# (d)
The financial condition of the applicant.
# (e)
A description of applicant’s operations.
# (f)
