License to distributor; application; contents

Sec. 3. (a) A person desiring to become licensed as a distributor in Indiana must file with the administrator a sworn application containing

the following information:

(1) The name under which the distributor will transact business in Indiana.

(2) The applicant's principal place of business.

(3) The name and complete residence address of the owner or the names and addresses of the partners, if the applicant is a partnership,

or the names and addresses of the principal officers, if the applicant is a corporation or association.

(4) Other information the administrator reasonably requires.

(b) A person may not operate as a distributor in Indiana without first satisfying the requirements set forth in IC 6-6-1.1-404

through IC 6-6-1.1-406 and IC 6-6-1.1-410, and without first being issued a license by the administrator under IC 6-6-1.1-
