TITLE 86: REVENUE CHAPTER I: DEPARTMENT OF REVENUE PART 105 ELECTRONIC FILING OF ILLINOIS INDIVIDUAL INCOME TAX RETURNS SECTION 105.300 GENERAL INFORMATION

Section 105.300 General Information a) Generally, previous applicants will be issued a Department information update form (EFS-15). Follow instructions included with the form and respond only when changes are necessary. b) New applicants must submit application Form IL-8633. c) Use only the official Form IL-8633 or a substitute form that duplicates the application in format, language, content, color and size.
