34 Tex. Admin. Code § 13.6 — Minimum Requirements for a Claim

A claim under Property Code, Chapter 74, Subchapter F, must identify:

# (1)

the name of each claimant;

# (2)

the mailing address for each claimant;

# (3)

the social security number or employer identification number of each claimant, or a statement that a claimant does not have such a number; and

# (4)

each specific property in the possession of the comptroller that is being claimed by reference to the unique property identification number assigned to each claimed property by the comptroller.
