Affidavit for correction of address or manufacturer's I.D. number on a license plate, registration certificate or title may be obtained from the Internal Administration Unit, Department of Revenue, Trinity-Washington Building, Atlanta, Georgia 30334, or any county tag agent. This form must be completed, signed by owner, notarized and supported by the license plate registration certificate or title in error along with a new, properly prepared application to the above Unit.
